Sample JHA: from the photo to the last signature

This is what 29Safety prints for a crew, filled out for a made-up job. Scroll and the page follows along. Switch to the plant floor or to Spanish any time.

Sample job hazard analysis, page 1 of 3
Page 1 of 3Storm drain trench, Round Rock, TX

It starts with where the work is

A photo of the spot, taken that morning, leads the document. Nobody guesses which trench or which line.

If someone gets hurt, it's on page 1

The address to give 911, who has first aid, where the AED is, and where to meet.

The risk matrix counts itself

Every hazard on page 2 lands in a box, before and after controls. Change a hazard or a control and the counts change with it.

Every step, what can go wrong, and what you do about it

Job steps in order, the OSHA rule behind each hazard, who makes sure it's done, and the risk before and after.

The crew signs before work starts

Names, signatures and times, on paper or on their phones.

Change it after it's signed

Something changes at 10:25? Open the JHA, fix the step, and it saves as the next revision with your name and the time. Someone joins the crew at 11:15? They go over it on their phone and sign. Earlier versions stay on file.

Take a control away and watch the risk climb

This is the same math the JHA uses. Each control you check lowers how likely the hazard is; severity stays where it is.

Before controls
Severity: first aidfatal
After controls
first aidfatal
